Community Care Coordinator

At Whiddon, we do what we do because we are passionate about our relationships with older Australians and providing the care and support that they need to continue their independent living for as long as possible. We have a large community network of home care employees, home care coordinators and senior community coordinators within our Home Care team, providing domestic, personal care and social support to our clients across NSW and QLD. The Community Coordinator position is a busy and varied role responsible for building, supporting and strengthening social, family & community networks. You will provide oversight of your clients care plans, undertake assessments and develop goals to assist your clients to live safely & independently in our community. You will be working with a supportive & collaborative team in a role which enables you to work with purpose each and every day, to really make a difference in people lives.
