Sheffield Community CAMHS (Child and Adolescent Mental Health Services) is dedicated to improving the mental health and wellbeing of young people across Sheffield. Our team delivers high-quality care and support to children and families, helping them through their mental health journeys. Under supervision of Registered Nurses, you will deliver care coordination services to young people and their families, enhancing access and collaboration throughout their care journey in CAMHS.
Job Responsibility:
Provide care coordination to young people and their families, ensuring the smooth access and communication across services
Collaborate with multidisciplinary professionals and external agencies to support the holistic care of young people
Be the lead contact for families, responding to telephone enquiries and providing support regarding patient care
Facilitate communication and information sharing between the multi-agency network, incorporating safeguarding, education, and transition issues into the therapeutic process
Requirements:
Knowledge and understanding of current government safeguarding legislation
Application of Health, Safety, and Risk management policies
Ability to develop good therapeutic relationships with clients
Competent in the use of IT systems including electronic clinical records
Excellent communication / interpersonal skills (verbal / written)
Excellent organisational skills and the ability to manage time effectively
Have access to a car or alternative transport to carry out home visits
Extensive working with families / safeguarding children / domestic abuse/ substance misuse/ mental health issues
Experience of working within a multidisciplinary team
Experience of working with children and young people
Experience of multi-agency working
RMN (Mental Health) or RLDN (Learning Disabilities)
Evidence of ongoing professional development
Degree in relevant subject
Current NMC registration
Ability to deal with sensitive and complex issues
Ability to use supervision and personal development positively and effectively
Nice to have:
Knowledge of the principles of clinical governance and its application
Demonstrates a knowledge of issues relating to children and young people and the impact it can have on mental health
Demonstrates an understanding of the need to use evidence-based therapies and how it relates to the post
In depth knowledge of safeguarding children, and factors which impact on parenting capacity
Experience of working in a child and adolescent mental health or related services
Worked in a service where agreed targets in place to demonstrate outcomes
