Job Responsibility:

  • Participate in initial assessments, undertaking carer/support person interviews
  • Participate in feedback appointments when client and family are informed about the results of the CDAMS Clinic assessments and diagnosis
  • Provide short term follow up of these clients
  • Assist clients and their carers to navigate the health service system
  • Provision of high quality, accessible, flexible and client-centred service

Requirements:

  • Degree in Allied Health, Nursing, Pharmacy or Psychology with relevant registration to your professional body
  • Experience in the field of cognitive impairment assessment and management
  • Demonstrated commitment to client centred care
  • Ability to communicate effectively in both written and verbal form
  • Ability to problem solve in a variety of complex situations
