Job Description:

South Tyneside Older Adults Community Treatment Team are looking for a full time Clinical Support assistant. This role will involve supporting a caseload of older adults within the community mainly in their own homes, who have various mental health problems including a diagnosis of dementia, psychosis, depression and anxiety. The role involves supporting the clinical members of the team to deliver evidence based treatment and to engage in collaborative working across the multi-agency team, as well as maintain the health and well being of this patient group.

Job Responsibility:

  • Assist in the delivery of care to service users who have a variety of needs, diverse presentations and a range of mental health conditions, including cognitive impairment, under the direction of a registered practitioner
  • Implement and evaluate care within a prescribed framework as a member of the care team ensuring that high quality, individualised care is delivered and that this promotes recovery and wellbeing and maximises independence
  • Complete Physical health checks including phlebotomy and ECG, in line with NICE guidelines
  • Support the team in providing excellent and effective person centred care
  • Provide compassionate care that is based on empathy, kindness, respect and dignity
  • Actively engage with supervision and appraisal process as identified in Trust policy

Requirements:

  • Senior Healthcare Support Worker or equivalent or working towards
  • Experience of working/ volunteering in an appropriate care setting
  • Experience of working with service users who have complex needs
  • Good verbal and written communication skills. ICT Functional skills
  • Good and effective time Management
  • Ability to work without direct supervision
  • Ability to meet the mobility and geographical requirements of the post

Nice to have:

  • Experience of working within a relevant clinical service
  • GCSE A-C in maths and English or equivalent
  • Previous experience in a community setting
  • Experience of older adults including complex needs and cognitive impairment
  • Understanding of person Centred Care
  • Phlebotomy and ECG Training
