Job Description

As a Thyme Care Oncology Social Worker, you'll be a vital resource for our members and their care network, offering comprehensive psychosocial and emotional support during their cancer journey via phone, email, text, and telemedicine.

Job Responsibility

  • Conduct comprehensive psychosocial assessments to identify patients' and families' needs
  • Address emotional concerns such as anxiety, depression, grief, adjustment, and coping mechanisms related to cancer diagnosis and treatment
  • Assess for and address financial toxicity, distress, changes in relationships/roles, custody planning, caregiver demands, advance care planning, and spiritual/existential distress
  • Assess, diagnose, and provide therapeutic interventions to address behavioral health needs
  • Providing immediate support and intervention in behavioral health crises and sudden changes in prognosis or end-of-life decisions
  • Identifying and connecting patients and families with relevant community resources when there are complex social or behavioral health needs
  • Collaborating with the healthcare team to ensure patients receive coordinated care and address any social or emotional barriers impacting treatment adherence
  • Advocating for patients' needs with insurance companies, healthcare providers, and other relevant agencies to access necessary treatment and support services
  • Leading support groups for cancer patients and their families, providing education on cancer-related topics, and facilitating peer-to-peer support

Requirements

  • Master's degree in Social Work from a school or program accredited by the Council on Social Work Education (CSWE)
  • Valid and current master’s level license in current practice state (i.e., LISW, LCSW)
  • Minimum three to five years of experience is preferred, working as a social worker in a health care setting
  • Oncology, palliative care, serious illness care and/or hospice experience is desired
  • An unrestricted license and a willingness to obtain additional state licenses as needed
  • Experience with video chatting, Google Suite, Slack, electronic health records, or comfort in using and learning new technology is vital

Nice to have

  • OSW-C or APHSW-C certification preferred
  • It would be exceptional if you have worked at a startup or tech-forward company
