Licensed clinician with current licensure to practice professionally within the state
Graduate of an accredited school
Two years management or supervisory experience in health care setting, preferably home care
Complies and maintains current CPR certification
Valid driver’s license and proof of insurance is required

Job Responsibility

Receives case referrals
Reviews available patient information to determine home care needs
Assigns appropriate clinicians to case
Reviews and evaluates each case
Reviews patient’s clinical diagnosis, medications, procedures and clinical course
Assists clinicians in establishing therapeutic goals and developing plan of care
Conducts or assists with quarterly record reviews
Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives

Job Responsibility

Manage and report on incidents, complaints, investigations, and patient safety outcomes
Support and oversee compliance with mandatory training, clinical audits, and quality improvement plans

Liaise with commissioners, regulators, and internal stakeholders on matters of quality and compliance
Prepare comprehensive reports for the Director of Care and Quality and the Executive Team
