The Clinical Pharmacist provides comprehensive pharmaceutical care services to patients, working collaboratively with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. The Clinical Pharmacist is responsible for medication order review, pharmacokinetic monitoring, drug information provision, patient education, and participation in clinical initiatives.
Job Responsibility:
Review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies
Monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity
Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals
Educate patients on their medications, including proper administration, potential side effects, and drug interactions
Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases
Participate in antimicrobial stewardship programs to promote the appropriate use of antibiotics and prevent antimicrobial resistance
Monitor for and report adverse drug reactions
Requirements:
Completion of a 4-year course at an accredited School of Pharmacy with a BS or Pharm. D. degree
At least two years of hospital pharmacy experience preferred or has completed an ASHP-accredited residency
Microsoft Office, Common Drug Information Software
Current CA Pharmacist Licensure required
Fire Safety card within 90 days of hire
