Job Description:

Clinical Pharmacist role at The Winterbourne Hospital in Dorchester and The Harbour Hospital in Poole, part of Circle Health Group. This is a full time role for 37.5 hours a week. Part time hours will be considered. The role holder will be required to cover a flexible shift pattern with no weekend or on call working. This is a dual site role with responsibilities split between both hospitals.

Job Responsibility:

Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation
Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations
Support safe prescribing through reconciliation, clinical screening, and dose adjustment (e.g., renal or hepatic impairment)
Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements
Promote safe and cost-effective use of medicines by supporting adherence to local and national policies

Requirements:

A master’s degree in pharmacy (MPharm or equivalent)
GPhC registration as a Pharmacist
Previous experience in a clinical hospital pharmacy role
Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation
Excellent communication and clinical decision-making skills
Ability to work autonomously and as part of a multidisciplinary team
Confident using electronic prescribing and medication administration systems (e.g., EPMA)
