Job Responsibility:

perform comprehensive patient assessments
create and implement personalised care plans
deliver evidence-based treatments to optimise patient outcomes
collaborate with multidisciplinary teams to provide holistic care

Requirements:

a current and valid GPhC registration
previous hospital experience in a ward-based setting
excellent clinical judgement and attention to detail
strong interpersonal and communication skills
a passion for delivering outstanding patient care
