Clinical Pharmacist

• Review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies
• Monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity
• Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals
• Educate patients on their medications, including proper administration, potential side effects, and drug interactions
• Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases
• Participate in antimicrobial stewardship programs to promote the appropriate use of antibiotics and prevent antimicrobial resistance
• Monitor for and report adverse drug reactions

• Completion of a 4-year course at an accredited School of Pharmacy with a BS or Pharm. D. degree
• At least two years of hospital pharmacy experience preferred or has completed an ASHP-accredited residency
• Microsoft Office, Common Drug Information Software
• Current CA Pharmacist Licensure required
• Fire Safety card within 90 days of hire
