Job Description:

Hunter AHP have an exciting opportunity for an experienced Locum Clinical Pharmacist to join a supportive GP surgery team in Reading. Duties include medication reviews, prescription management, and providing expert advice on medicine optimisation.

Job Responsibility:

Conducting medication reviews (face-to-face and telephone)
Supporting repeat prescription management and medicines reconciliation
Providing expert advice on medicines optimisation
Running long-term condition clinics (where appropriate)
Supporting QOF, IIF, and other practice-led targets
Working as part of the multidisciplinary GP practice team

Requirements:

Minimum 2 years’ experience working as a Clinical Pharmacist in a GP surgery
GPhC registered pharmacist
Independent Prescriber qualification not essential
Confident working autonomously within primary care settings
