Hunter AHP are currently supporting an NHS Trust in Lancashire with the recruitment of an experienced Clinical Pharmacist for an ongoing locum contract. This postholder will be part of a well-established pharmacy team delivering high-quality care across multiple wards in a dynamic and supportive environment.
Job Responsibility:
Provide clinical pharmacy services to a range of inpatient wards, including medical and/or surgical specialties
Conduct medicines reconciliation and reviews for newly admitted patients
Deliver pharmaceutical care planning and optimisation of medicines use
Offer clinical advice to multidisciplinary teams, including consultants, nurses, and allied health professionals
Monitor patient progress and make evidence-based interventions to improve therapeutic outcomes
Ensure safe discharge by reviewing TTOs and liaising with community providers as needed
Support antimicrobial stewardship and compliance with local formulary and guidelines
Contribute to audits, service improvement, and governance activities within the pharmacy department
Requirements:
GPhC-registered Pharmacist
Minimum of 6–12 months’ recent NHS ward-based experience
Strong clinical knowledge and confidence working independently on the wards
Experience in medicines reconciliation, discharge planning, and multidisciplinary communication
Ability to work well within a team and manage time effectively
Commitment to patient safety and evidence-based care
