Job Description:

Hunter AHP are currently supporting an NHS Trust in Lancashire with the recruitment of an experienced Clinical Pharmacist for an ongoing locum contract. This postholder will be part of a well-established pharmacy team delivering high-quality care across multiple wards in a dynamic and supportive environment.

Job Responsibility:

  • Provide clinical pharmacy services to a range of inpatient wards, including medical and/or surgical specialties
  • Conduct medicines reconciliation and reviews for newly admitted patients
  • Deliver pharmaceutical care planning and optimisation of medicines use
  • Offer clinical advice to multidisciplinary teams, including consultants, nurses, and allied health professionals
  • Monitor patient progress and make evidence-based interventions to improve therapeutic outcomes
  • Ensure safe discharge by reviewing TTOs and liaising with community providers as needed
  • Support antimicrobial stewardship and compliance with local formulary and guidelines
  • Contribute to audits, service improvement, and governance activities within the pharmacy department

Requirements:

  • GPhC-registered Pharmacist
  • Minimum of 6–12 months’ recent NHS ward-based experience
  • Strong clinical knowledge and confidence working independently on the wards
  • Experience in medicines reconciliation, discharge planning, and multidisciplinary communication
  • Ability to work well within a team and manage time effectively
  • Commitment to patient safety and evidence-based care
