As our Clinical Pharmacist, you'll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury. Every day, you'll build trust in these interactions by reviewing every prescription in your queue. We never rely entirely on the computer to check for safety; we always use an expert for the final check. That's you. You'll review prescriptions for potential drug interactions, correct dosing, patient allergies or anything that looks unusual in your opinion. To excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for our patients. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.
Job Responsibility
Ensure safe compounding and dispensing of chemotherapy infusions and nonhazardous medications
Review and verify medication orders for appropriateness, safety, efficacy, and considering patient-specific data like age, weight, diagnosis, allergies, and lab values
Monitor patient-specific pharmacotherapeutic regimens, identifying potential and actual drug-related problems, and making recommendations or adjustments to therapy in collaboration with physicians and the healthcare team
Provide accurate drug information to healthcare professionals and patients
Maintain accurate and complete documentation of patient assessments and clinical interventions with the use of Cerner systems for electronic health records
VARIOUS WORKSITES – Regular travel for clinical coverage within Phoenix to other Dignity locations, including Dignity Health Cancer Institute at St. Joseph’s, as determined by business needs
Requirements
Doctor of Pharmacy or closely related
36 months in job offered, or closely related
Ability to provide patient counseling regarding prescription and over the counter (OTC) medications
Ability to provide oversight regarding pharmacy operations and assist with triaging issues within the department
Experience with therapeutic drug monitoring and pharmacokinetic dosing for vancomycin, aminoglycosides and anticoagulants for optimized medication management and therapy for patients
Knowledge of sterile compounding practices and IV admixture oversight of chemotherapy, biologics, and nonhazardous medications
Knowledge of oncology treatment protocols such as NCCN guidelines, in tandem with the ability to recommend chemotherapy orders with accuracy, safety, and appropriateness
Experience receiving and transcribing prescription data into computer systems including Epic to maintain electronic health records for patients
Must have an Arizona Pharmacist License from the Arizona State Board of Pharmacy
Must have or be able to obtain a Basic Life Support Certification from an accredited American Heart Association
