Job Description:

Job Responsibility:

Conduct detailed patient assessments and clinical checks
Develop and implement tailored treatment plans
Provide expert medication advice to clinical teams
Work closely with a multidisciplinary team to enhance patient outcomes
Maintain accurate, up-to-date clinical records

Requirements:

Current and valid GPhC registration as a Pharmacist
Previous hospital pharmacy experience
Confidence working independently across multiple ward types
Strong communication skills and a team-focused approach
A commitment to delivering safe, effective care to every patient
