Job Description:

Join our GP Surgery client’s team in Kent as an IP Clinical Pharmacist. Enhance patient care within a primary care setting by conducting medication reviews, managing long-term conditions, and improving prescribing processes. Contribute to audits and safety initiatives while working closely with GPs and MDTs.

Job Responsibility:

Conduct structured medication reviews and optimise prescribing
Work closely with GPs and the wider MDT to manage long-term conditions
Provide prescribing support to improve clinical outcomes
Support safe and effective repeat prescribing processes
Contribute to clinical audits and patient safety initiatives

Requirements:

A current and valid GPhC registration
Independent Prescribing Qualification (required)
Previous experience in GP or Primary Care settings
Strong clinical knowledge and patient-facing communication skills
The ability to work independently and as part of a team
