Ensures the safe and effective use of medications by providing medication order verification while assessing for optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services. Monitors inpatient drug therapies when appropriate, and works with relating medical staff to ensure the best care possible is being given.
Job Responsibility:
Taking patient medication history
Prioritize level of care based on critical need
Participate in interdisciplinary patient care rounds and communicate and follow up with the clinical team
Prevent, detect and resolve medication-related problems to make evidence-based patient-centered medication therapy recommendations
Verify patient medication orders for appropriateness and accuracy
Document critical intervention
Provide drug information for patients and all health care team
Provide discharge medication teaching upon request
Report medication errors and ADRs
Participate collegially in the development of new programs, services, and practices, educational activities of the clinical pharmacy department, and mentor clinical pharmacy interns
Establishes regular meetings with clinical pharmacy manager/chief pharmacist
Identify potential technical or patient care issues and work with the clinical pharmacy manager/chief pharmacist to make recommendations to avoid issues
Monitor newly added medications upon request
Participates and provides consultation to hospital committees, such as Pharmacy and Therapeutics (P&T), clinical team committee, and antibiotic committee when pharmacotherapy issues are discussed
Requirements:
Bachelor of Pharmacy, with Post Graduate degree in Clinical Pharmacy
One year as Clinical Pharmacist from a reputable hospital
