Clinical Pharmacist

• Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation
• Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations
• Support safe prescribing through reconciliation, clinical screening, and dose adjustment
• Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements
• Promote safe and cost-effective use of medicines by supporting adherence to local and national policies
• Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity
• Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance

• A degree in Pharmacy MPharm, BSc or BPharm
• GPhC registration as a Pharmacist
• Previous experience in a clinical hospital pharmacy role
• Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation
• Excellent communication and clinical decision-making skills
• Ability to work autonomously and as part of a multidisciplinary team
• Confident using electronic prescribing and medication administration systems (e.g., EPMA)
