Clinical Pharmacist

• Meticulously review every prescription in your queue for safety, critically assessing for potential drug interactions, correct dosing, patient allergies, or any irregularities
• Authorize prescriptions for the pharmacy technician, performing a final check before dispensing medication to the nursing station
• Demonstrate ability to document clinical activities and interventions such as consults, patient teachings and other patient specific information as defined by policy and procedure
• Interpret medication orders and/or prescriptions for drug interactions, appropriate drug selection and appropriateness
• Accurately enter / verify the order or prescription into the computer system, labels, and dispense medications in a timely manner
• Verify and check medications prepared by other pharmacy personnel
• Communicate drug information to patients and health professionals
• Resolve drug related problems by consulting with other health care professionals to achieve appropriate patient outcomes
• Clarify unclear orders written by a licensed provider with the licensed provider
• Assist the licensed provider in appropriate drug selection and therapeutic regimens
• Achieve and maintain an adequate basic knowledge base to provide clinical services for all age groups served
• Provide basic clinical services as required by policy and procedure
• Maintain a working knowledge of automated systems
