Job Description

Under general supervision, is responsible for the delivery and documentation of pharmaceutical care provided to patients using the medication use process (selecting, prescribing, dispensing, administration and monitoring of medications).

  • Review every prescription in your queue
  • Review prescriptions for potential drug interactions, correct dosing, patient allergies or anything that looks unusual
  • Document clinical activities and interventions such as consults, patient teachings and other patient specific information as defined by policy and procedure
  • Interpret medication orders and/or prescriptions for drug interactions, appropriate drug selection and appropriateness
  • Accurately enter/verify the order or prescription into the computer system, labels, and dispenses medications in a timely manner
  • Verify and check medications prepared by other pharmacy personnel
  • Communicate drug information to patients and health professionals
  • Resolve drug related problems by consulting with other health care professionals to achieve appropriate patient outcomes
  • Clarify unclear orders with the licensed provider
  • Assist the licensed provider in appropriate drug selection and therapeutic regimens
