Job Description

CareGivers America, a Modivcare Personal Care Service, is looking for an RN/LPN responsible for providing high-quality services to our home care private duty clients.

Job Responsibility

  • Conduct initial home visits to assess patient needs
  • Conduct starts of care, resumption of care, assessments, and supervisory visits/re-visits
  • Develop Plan of Care for each patient and continuously monitoring the need to revise or reevaluate based on patient's progress
  • Document daily clinical notes and progress updates on the patient's clinical record
  • Communicate with a patient’s physician and other healthcare professionals to ensure delivery of appropriate patient care
  • Provide skilled nursing care as necessary

Requirements

  • RN/LPN with at least 1 year of experience preferred
  • 1 year of Home Care experience preferred
  • New grads welcomed

  • Plan individualised 1:1 Sessions for Residents as appropriate and engage with ongoing life story work to help inform the activity programme and person-centred care planning
  • Manage the activity budget in conjunction with the Home Manager to ensure the provision of resources, outings and entertainment and undertake fundraising opportunities to supplement as appropriate
  • Maximise the use of resources that are available to support with provision of meaningful engagement opportunities. Ensure risk assessments are completed for all activities and outings and reviewed as appropriate
  • Act as a Dementia Ambassador at your Location
  • Support all Colleagues to become Dementia Friends
  • Offer Signposting to family members to Admiral Nurse services, Local carer groups, tide, Alzheimer's Society etc
  • Maintain stock of Dementia Publications
