This specialist role supports the early identification, assessment, and management of patients with Acute Kidney Injury and Sepsis across the organisation. You will work autonomously across front-door and inpatient areas, delivering expert clinical assessment, initiating timely interventions, and guiding teams in the use of the SHOUT bundle and Sepsis Six. You will educate and support staff in recognising deterioration, contribute to quality improvement initiatives, and promote best practice in line with NICE guidance. The role involves close multidisciplinary collaboration, providing expert advice on fluid management, medications, and treatment planning. You will also support safe discharge planning, including telephone follow-ups for AKI patients, and play a key part in delivering a reliable seven-day service.
Job Responsibility:
Provide specialist clinical assessment and management for patients with Acute Kidney Injury
Work proactively across front-door areas and inpatient wards to identify deteriorating patients
Support staff in the early recognition of sepsis using the Sepsis Six tool
Deliver high-quality education and training to multidisciplinary teams
Lead and contribute to quality improvement projects relating to AKI and sepsis care
Work collaboratively with ward teams, the Emergency Department, and specialty services
Provide expert advice on fluid management, medication, and treatment plans
Support the delivery of a seven-day service
Guide safe discharge planning and follow-up for complex AKI patients
Act as a clinical role model
Participate in policy development, pathway refinement, and implementation of NICE-aligned best practice
Requirements:
First level NMC registration
Teaching and Assessing Qualification/ ENB 998/ City and Guilds 730 or equivalent
Bachelors Degree (or evidence of level 3 learning supplemented by diploma level specialist training/experience/ education)
Demonstrable post qualification experience within an acute environment
High standard of nursing care and clinical knowledge
Ability to support patients and family members
Evidence of continued professional development
Computer Literacy
Nice to have:
Relevant clinical course to enable history taking, and clinical examination, diagnostic reasoning, and differential diagnosis
Experience of managing acutely/ critically ill patients
Experience of delivering the Sepsis 6 and/or SHOUT bundles
Participation in research and /or audit
Experience of managing change and developing services
Demonstrable experience of delivering teaching/ training
