We have an exciting role to join our innovative and publicly acclaimed Palliative Hub Team as a Clinical Nurse Specialist. Our Palliative Hub service provides specialist palliative and end of life care to any adult (age 18 or over) who is registered with a GP within the Cambridge and Peterborough Integrated Care Board via a telephone advice line and through NHS 111 service. We help end of life patients to receive care and support throughout the Cambridgeshire area. The enhanced services provide a single point of access (SPA) through a centralised hub for: patients, families, carers, health and social care professionals. The aim of the telephone advice line is to support patients in the community to remain in their usual place of residence for as long as it is appropriate and safe to do so.
Job Responsibility:
Holistically assess, plan, implement and review therapeutic interventions of patients who contact the palliative Hub advice line by telephone, helping to avoid unnecessary hospital admissions

Visit the hospice at least on a monthly basis
