Job Description:

We are seeking an experienced and motivated Clinical Nurse Consultant (CNC) to provide expert clinical leadership across acute and subacute aged care at home beds (Acute Aged Care and GEM at Home). This specialist role supports the delivery of high-quality, evidence-based care for older people admitted to "at home" bed substitution program, with a strong focus on optimising patient outcomes and improving service quality. The CNC provides advanced nursing consultation through direct patient assessment, telephone support, and telehealth where appropriate, and works closely with the multidisciplinary team to ensure coordinated, person-centred care.

Job Responsibility:

  • Provide advanced clinical expertise and consultation for older people in at home bed substitution programs
  • Lead and support comprehensive assessment, care planning, implementation, monitoring, and review of care
  • Collaborate with multidisciplinary teams to optimise patient outcomes and reduce avoidable complications and length of stay
  • Ensure evidence-based practice and best-practice standards are implemented and maintained across services

Requirements:

  • Have postgraduate qualifications and/or significant experience in aged care, acute care, subacute care, or gerontology
  • Have worked at senior clinical role CNS or above
  • Have demonstrated advanced clinical expertise
  • Strong communication, consultation and collaborative practice skills
  • Have a valid Victorian drivers license
