An opportunity has arisen for a highly skilled, enthusiastic and motivated Occupational Therapist to join the management team of Integrated Care across Greater Nottingham. FDIDT are an in-reach multidisciplinary team made up of AHPs, Nurses and Assistant Practitioners. This Nottingham CityCare Partnership service provides innovative, successful and patient focussed outcomes for those that present in the emergency and urgent pre admission areas at Nottingham University Hospitals QMC site. Your role within the busy and dynamic team will be to provide a comprehensive holistic assessment of need with the aim of preventing avoidable hospital admissions. The successful candidate will be able to work within the MDT as an autonomous practitioner, undertake comprehensive assessments, and complete profession specific assessment and evaluation as required, to identify the appropriate discharge plans for adults with a wide variety of complex physical and mental health needs. Job Responsibility:
Provide a comprehensive holistic assessment of need with the aim of preventing avoidable hospital admissions
Work within the MDT as an autonomous practitioner
Undertake comprehensive assessments, and complete profession specific assessment and evaluation as required
Identify the appropriate discharge plans for adults with a wide variety of complex physical and mental health needs
Undertake comprehensive assessment and intervention to clients within the Pre-admission areas
Facilitate the clients discharge or appropriate pathway identification
Manage a caseload that consists of a wide range of cases for people presenting multi pathologies
Perform holistic assessments and a range of skills within a competency framework
Complete initial assessments and follow ups under the guidance and supervision of the Clinical staff
Utilise community knowledge and skills whilst assessing and recommending appropriate discharge support
Plan, develop and evaluate goal focussed rehabilitation
Prioritise workload, manage their time effectively, and work flexibly within the team
Participate in staff development programmes
Be professionally and legally responsible and accountable for all aspects of your work
Gain valid informed consent and identify clients who lack capacity
Undertake comprehensive assessment and evaluation of the patients Physical and Mental health needs
Use clinical reasoning and a wide range of skills and knowledge to devise specialised programmes of care
Coordinate the safe and timely discharge process from a variety of settings
