Clinical In-reach Occupational Therapist

An opportunity has arisen for a highly skilled, enthusiastic and motivated Occupational Therapist to join the management team of Integrated Care across Greater Nottingham. FDIDT are an in-reach multidisciplinary team made up of AHPs, Nurses and Assistant Practitioners. This Nottingham CityCare Partnership service provides innovative, successful and patient focussed outcomes for those that present in the emergency and urgent pre admission areas at Nottingham University Hospitals QMC site. Your role within the busy and dynamic team will be to provide a comprehensive holistic assessment of need with the aim of preventing avoidable hospital admissions. The successful candidate will be able to work within the MDT as an autonomous practitioner, undertake comprehensive assessments, and complete profession specific assessment and evaluation as required, to identify the appropriate discharge plans for adults with a wide variety of complex physical and mental health needs. The Teams Mission Statement is to provide a gold standard assessment and review of patient needs that allows ongoing treatment to be provided by the right service, at the right time, in the right place without delay or duplication.
