We’re Hiring: Full-Time RN Case Manager – Pediatric Home Health. CareGivers America – NEPA, a Modivcare Home Care company, is seeking a skilled and mission-driven Registered Nurse with a passion for pediatrics to join our Home Health team as an RN Case Manager. Your Nursing Skills Can Change Lives — Right at Home. Every day, you’ll have the privilege of helping pediatric patients recover, remain independent, and live with dignity in the comfort of their own homes. You’ll be the trusted point of contact for children and their families, guiding care from start to finish while leading a compassionate care team.

Job Responsibility:

  • Complete comprehensive patient assessments, including OASIS data collection, within established timelines
  • Collaborate with physicians, other healthcare professionals, patients, and families to develop individualized pediatric care plans
  • Provide direction and supervision to LPNs and Home Health Aides, ensuring quality and effective care delivery for children
  • Monitor clinical documentation for accuracy, timeliness, and compliance with all regulatory standards
  • Communicate regularly with pediatric patients’ families and care teams to coordinate services and address changing needs
  • Participate in quality improvement initiatives and staff education
  • Promote a safe and respectful care environment for patients and colleagues

Requirements:

  • Active PA RN License (Required)
  • BSN Preferred
  • 2+ years of professional nursing experience (pediatric or home health experience preferred)
  • Strong leadership, communication, and organizational skills
  • Ability to manage multiple priorities and work independently
  • Valid driver’s license
  • Current CPR certification
