Job Description:

As our Clinical Exercise Physiologist CRT, you will collaborate with a multi-disciplinary team to provide comprehensive lifestyle modification services for individuals with chronic diseases. Every day you will be responsible for developing and implementing treatment plans for your own caseload. To be successful in this role, you will actively participate in clinical and program decisions made by the Lifestyle Management Team.

Job Responsibility:

  • Collaborate with a multi-disciplinary team to provide comprehensive lifestyle modification services for individuals with chronic diseases
  • Develop and implement treatment plans for your own caseload
  • Actively participate in clinical and program decisions made by the Lifestyle Management Team
  • Utilize the AIDET principle to communicate with others, with a focus on patients and their families
  • Accurately assess client status and develop comprehensive treatment plan in collaboration with the Lifestyle Management Team
  • Establish patients on clinical pathway
  • Provide monitored exercise therapy to clients in the Lifestyle Management Program (i.e. with diagnosis of heart disease, diabetes, COPD, etc.)
  • Observe, record and report potentially dangerous signs and symptoms to physician and provides documentation to direct supervisor
  • Adjust treatment plan in accordance with established protocols, co-morbidities, client goals/needs and medical history
  • Monitor progress and update treatment/care plan prn
  • Actively participate in clinical team meetings to review current case load
  • Assist in and direct the education process of clients and their families regarding their disease, process, progress, and treatment plan
  • Maintain current clinical data, insurance information and program records on each client
  • Keep accurate patient medical records and timely entry of data entry in patient chart

Requirements:

  • Masters Of Science or advanced degree in Exercise Physiology or related field
  • One year experience in lifestyle management, exercise training, prescription and supervision of medical patients, upon hire
  • Basic Life Support - CPR, upon hire
  • Advanced Cardiac Life Support, upon hire

Nice to have:

  • Two years experience
  • Certified Exercise Specialist, upon hire
  • Certified Personal Trainer/Exercise Speciast, upon hire
