Job Responsibility

Provides timely, comprehensive, evidence-based nutritional care and education to patients and families
Participates in the training of medical staff, dietetic interns and peers
Ensures best practices by regularly reviewing various literature and changing policies/practice as needed
Acts in a manner which is highly responsive to patients' needs and promotes and supports the highest quality nutritional care
Provides ongoing nutritional assessment, monitoring, evaluation, and education to patients and families
Develops and implements individualized nutrition care plans for patients across the lifecycle, from infants to the elderly, as assigned
Integrates evidence-based research in the education, practice and management of patients
Effectively communicates nutrition plans and rationale to team members verbally and in the medical record
Demonstrates the ability to utilize various HIPAA secure patient communication modalities such as Zoom, My Chart, etc
Provides education on medical nutrition therapy and food-drug interactions to patients to patients, their families and significant others

Job Responsibility

Educate patients and their families and / or significant others in diabetes and nutrition education needed per visit type
Implement and document teaching plan, educational interventions, and follow-up plan based on the individualized assessment for patients and their families and / or significant others per current program educational curriculum or current evidence based recommendations
Collaborate with patients and their families and / or significant others in establishing mutual learning goals and behavior change goals
Document evaluation of outcomes and revises interventions and follow-up plan for patients and their families and / or significant others
Assure education is based on current program curriculum, ADA guidelines for a nutrition care plan, and input from physicians
Communicate education and follow-up recommendations to physicians and other health care providers within the multidisciplinary team
Bill third party payers for diabetes outpatient self-management and training and medical nutrition therapy, utilizing appropriate coding guidelines
Serve as a resource person and mentor to staff related to clinical knowledge of diabetes management and nutrition therapy
Act as a resource to other departments and disciplines within AMG and community

Job Responsibility

Monitor vital signs, labs, and other medical data to ensure medical stability
