Job Description:

Help elevate patient care by serving dual eligible populations utilizing best-in-class operating and clinical models. Collaborate with members, providers, and community organizations to address the full continuum of our members' health care and social determinant needs.

Job Responsibility:

Facilitate delivery of appropriate benefits and healthcare information
Develop, implement, and support Health Strategies
Use collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy
Meet individual's and family's comprehensive health needs through communication and available resources

Requirements:

3-5 years of direct clinical practice experience post masters degree
Unencumbered Behavioral Health clinical license in the state where they work, LCSW or LCPC
Confidence working at home/independent thinker
Ability to travel within a designated geographic area for in-person case management activities
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Proficiency with standard corporate software applications
Efficient and Effective computer skills including navigating multiple systems and keyboarding
Reliable transportation required

Nice to have:

Case management and discharge planning experience
Managed care/utilization review experience
Crisis intervention skills
