Job Description:

Clinical Case Manager Behavioral Health position with Aetna's Long-Term Services and Supports (LTSS) team, a field-based position requiring 50-75% travel to meet with members face to face in Cook County IL and surrounding areas. This position holds a full caseload to manage waiver members and requires in person quarterly visits with members to meet contractual requirements.

Job Responsibility:

  • Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education
  • Conduct comprehensive evaluation of referred members' needs/eligibility
  • Identify high risk factors and service needs that may impact member outcomes
  • Coordinate and implement assigned care plan activities and monitors care plan progress
  • Enhance medical appropriateness and quality of care
  • Utilize negotiation skills to secure appropriate options and services
  • Provide coaching, information and support to empower members
  • Help members actively participate with their provider in healthcare decision-making
  • Utilize case management and quality management processes

Requirements:

  • Active and good standing Illinois BH clinical license – specifically LCSW, LCPC, or LMFT
  • Must reside in Cook County IL or Surrounding Areas (Applicable zip codes: 60402, 60455 60632, 60638, 60609, 60636, 60629, 60804)
  • Willing and able to travel up to 50%-75% of their time to meet with members face to face
  • Reliable Transportation required
  • Minimum 2 years of experience in behavioral health, social services or human services field
  • Minimum 2 years of case management experience
  • Master's degree in social services, human services or behavioral health required
