Job Description:

Clinical Case Manager Behavioral Health position at CVS Health. This is a remote role requiring 75% travel to meet in-person with members around the service delivery area including Arlington and Southeast Dallas, Texas. The role involves utilizing advanced clinical judgment to facilitate appropriate member healthcare through assessment and care planning, direct provider coordination, and coordination of psycho social wraparound services.

Job Responsibility:

Utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare
Conducts comprehensive assessments of referred member's needs/eligibility
Performs crisis intervention with members experiencing behavioral health or medical crisis
Provides crisis follow up to members
Application and/or interpretation of applicable criteria and clinical guidelines
Identifies and escalates quality of care issues
Provides coaching, information and support to empower members
Develops and monitors established plans of care
Completes face to face visits and monthly phone contact with members

Requirements:

Direct clinical practice experience post masters degree
Minimum of a Master's degree in Behavioral/Mental Health or related field
75% travel required to meet in person with members
Must possess reliable transportation
Unencumbered Behavioral Health clinical license in the state where they work (LCSW, LPC, LMSW, LMFT)
