Job Description:

Field-based Clinical Case Manager position with Aetna's Long-Term Services and Supports team, providing care coordination and support for waiver members through face-to-face visits and comprehensive case management services.

Job Responsibility:

Facilitate appropriate healthcare outcomes for waiver/LTSS members
Provide care coordination, support and education for members
Conduct comprehensive evaluation of referred members' needs/eligibility
Identify high risk factors and service needs
Coordinate and implement assigned care plan activities
Monitor care plan progress
Consult with case managers, supervisors, Medical Directors
Identify and escalate quality of care issues
Utilize negotiation skills to secure appropriate options and services
Provide coaching, information and support to empower members
Help members participate with their provider in healthcare decision-making

Requirements:

Active Illinois BH clinical license – specifically LCSW, LCPC, or LMFT
Must reside in Will, Kane or DuPage County IL or Surrounding Areas
Willing and able to travel up to 50%-75% of their time to meet with members face to face
Reliable Transportation required
Minimum 2 years of experience in behavioral health, social services or human services field
Minimum 2 years of case management experience
Bachelor's Degree in social services or related field required

Microsoft Office experience
Master's Degree
