Job Description

The Clinical Case Manager Lead, under the direction of the Clinical Case Management Manager, is a department leader who collaborates with the healthcare team to ensure safe, efficient, evidence based care/service to patients of all ages and their families. Primary role is to lead, motivate and train the case management team. Responsible for developing, implementing and updating department training resources to reflect changes within case management role. Organizes new employee orientation by scheduling preceptors, intermittent follow-up and providing oversight of orientation and competency of staff. Educates all staff (including weekend and PRN team) as it relates to department functions, processes, regulatory requirements and systems appropriate to the position. Trains and mentors new case management staff. Creates and maintains a supportive work environment for the entire team.

  • Work with the manager and human resources in performance management counseling and discipline of staff

Requirements

  • Bachelors Degree in Nursing OR Masters Social Work OR 2 or more years case management/care coordination experience
  • Licensed Registered Nurse (LRN) - Single State - State Board of Nursing Registered Nurse in State of Kansas OR Licensed Master Social Worker (LMSW) - State Division of Professional Registration in State of Kansas OR Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
