Job Responsibility

Study inpatient records and determine their authenticity
Review patients’ documentation to determine diagnosis, co-morbidities, and complications
Verify that each code assigned is accurate and correlates to the condition and diagnosis provided in the source document
Serve as a resource for coding related queries after verifying authenticity of inquirer
Ascertain that quality and integrity of coding including accurate determination of principle diagnosis, secondary diagnosis, and PCS codes
Responsible for reviewing patient medical records in the inpatient setting to capture accurate illustration of the severity of illness and facilitate proper coding
Facilitate appropriate coding that reflects the severity of the patient’s illness along with expected risk of mortality and complexity of care required
Documentation of discharge diagnoses and co-morbidities is a comprehensive reflection of the patient’s clinical status and care
Use advanced knowledge of disease processes (pathophysiology), medications, and have critical thinking skills to analyze current documentation to identify gaps
Recognize opportunities in concurrent and retrospective inpatient clinical medical documentation to support quality and effective coding

Job Responsibility

Assign DRG Codes: Accurately assign DRG codes to inpatient records using ICD-10- CM/PCS coding systems based on clinical documentation and physician notes. Ensure codes reflect the correct diagnosis, procedures, and the overall complexity of care.
Clinical Documentation Review: Review and analyze medical records to verify diagnoses, procedures, and treatments. Work with physicians and healthcare providers to clarify and improve clinical documentation when needed for proper coding.
DRG Assignment: Utilize the DRG methodology to ensure accurate and consistent DRG assignment based on the severity of illness (SOI) and risk of mortality (ROM), among other factors. Apply coding conventions and guidelines as per CMS (Centers for Medicare & Medicaid Services) and payer requirements.
Continuous Education: Keep up to date with coding guidelines, coding technology, and industry changes related to DRGs, including changes in ICD-10-CM/PCS, federal regulations, and insurance payer policies.
Billing Support: Work closely with the billing department to resolve coding issues and ensure that all claims are processed correctly and promptly for reimbursement. Ensure timely submission of all inpatient claims for accurate payment processing

Candidates holding CCS/CIC with hospital coding experience are preferable
