This service operates through Pathways Hubs and provides day services for young people.

Job Responsibility:

  • Person-Centred Care and Support: Delivering high-quality, person-centred care to improve and maintain an individual's health, well being, and happiness
  • Therapeutic Activities: Offering a wide range of enriching and multi-sensory therapeutic interventions
  • Meaningful Activities and Education: Facilitating educational, social, and enjoyable activities
  • Specialist Support: Working closely with healthcare professionals (e.g., Physiotherapists, Occupational Therapists, Speech and Language Teams) and undertaking specialist tasks
  • Focus on Outcomes: Supporting individuals to develop skills, promote self-confidence, build friendships, and work towards goals related to employment, independent living, community inclusion, and health
