Job Responsibility:
Provide high-quality, resident-centered care
Assist with Activities of Daily Living (ADLs): Hygiene, grooming, dressing, toileting, incontinence and catheter care, repositioning, skin care, and comfort measures
Ensure Resident Safety & Environment: Responding to call lights and alarms, fall prevention, low beds and safety mats, restraint assistance (as directed), and maintaining a clean, safe unit
Provide Nutrition Support: Meal delivery, tray setup, feeding assistance, nourishments, and honoring resident preferences
Provide Rehabilitative Support: Range of motion exercises, ambulation, transfers, use of assistive devices, and proper body alignment
Observation & Communication: Monitor and promptly report changes in physical, emotional, or psychosocial status
Documentation: Accurate and timely charting per facility protocol
Requirements:
Active NYS Nursing Assistant Certification
BLS Certification (AHA or RQI only)
Minimum 1 year of CNA experience in Long-Term Care
