Active, unencumbered CNA license in good standing
Current CPR certification
Minimum one year working with older adults or individuals with complex care needs
Valid driver’s license, proof of insurance, and reliable transportation
Medical clearance for communicable diseases and up-to-date immunizations prior to direct participant contact

Job Responsibility

Assist participants with activities of daily living (bathing, dressing, grooming, oral care, eating, toileting, hand and foot nail care)
Support mealtime needs, including feeding assistance when necessary
Help with laundry and light household tasks to maintain a clean, safe environment
Safely assist participants entering and exiting program vehicles
Monitor and promptly report changes in participants’ conditions to nursing staff
Contribute to a positive, supportive home environment for each participant
Perform other care-related duties as needed

Job Responsibility

Provide personal care assistance in the Day Center setting according to participant care plans, including support with activities of daily living (ADLs) such as grooming, dressing, toileting, ambulation, and feeding as needed
Assist participants during arrival and departure, ensuring safe transfers to and from vehicles and their homes
Support Transportation staff with participant care needs during transit and at medical appointments
Monitor and report changes in participants’ condition, behavior, or family situation to the PACE clinical staff promptly
Assist with group and individual Day Center activities during downtime, promoting engagement, socialization, and overall well-being
Adhere to infection control protocols, including universal precautions and the proper use of PPE
Ensure compliance with federal, state, and organizational guidelines for participant care
Complete documentation accurately and in a timely manner

Term-time only, with a full statutory annual leave entitlement of 5.6 weeks (inclusive of 5.6 weeks) paid in addition to the working weeks
There is no requirement to work during school holidays, although overtime may be available that will be paid in addition to your contractual earnings
