Job Description:

Blue Ridge Independence at Home is a Program of All-Inclusive Care for the Elderly (PACE). Our interdisciplinary team seamlessly blends expertise, compassion, and dedication to ensure each individual receives the preventative, primary, acute, and long-term health and personal care services they need. From clinicians to therapists, personal care aides to care coordinators, every team member plays a crucial role in delivering unparalleled care. Together, we prioritize the health, happiness, and independence of those we serve. We are seeking a compassionate Certified Nursing Assistant (CNA) to join our PACE Home Care team. In this role, you will provide essential care and assistance to participants in their homes, helping them maintain independence, comfort, and dignity. You will be an integral part of our care team, ensuring participants’ daily needs are met while fostering trust, safety, and respect.

Job Responsibility:

  • Assist participants with activities of daily living (bathing, dressing, grooming, oral care, eating, toileting, hand and foot nail care)
  • Support mealtime needs, including feeding assistance when necessary
  • Help with laundry and light household tasks to maintain a clean, safe environment
  • Safely assist participants entering and exiting program vehicles
  • Monitor and promptly report changes in participants’ conditions to nursing staff
  • Contribute to a positive, supportive home environment for each participant
  • Perform other care-related duties as needed

Requirements:

  • High school diploma or equivalent
  • Active, unencumbered CNA license in good standing
  • Current CPR certification
  • Minimum one year working with older adults or individuals with complex care needs
  • Valid driver’s license, proof of insurance, and reliable transportation
  • Medical clearance for communicable diseases and up-to-date immunizations prior to direct participant contact
