Job Responsibility

Under the direction of the Manager of Community Support Services (CSS), the Personal Support Worker (PSW) will provide a range of in-home services and support to eligible, high-risk seniors
A focus is placed on providing client centered, client directed and community-based care
As directed by the service plan, PSW will support clients by assisting them to achieve the highest degree of health and independence possible through the promotion and support of activities for daily living
Assist and support clients with activities for daily living as determined by their service plan
Document all care in the CSS Electronic Medical Report (EMR)
Assist and support clients to access community services and resources that relate to their goals and needs
Assist in coordination of services including transportation services, booking, and confirming health and wellness appointments, pickup of medications, ensuring food security, etc
Assist clients with range of motion regiments found in the care plan as directed by Occupational Therapists and Physiotherapists and promote community-based exercise programs tailored to seniors, as instructed
Provide well-being checks and services that aid in preventing the negative effects of social isolation to ensure client health and safety
Support clients with medication compliance
