The Riverside Community Stroke Rehabilitation (CSR) Rehabilitation Technician will support the therapists in providing person-centered and coordinated care for stroke rehabilitation. Delivered in a setting that best meets the needs of stroke patients who have experienced a stroke within the last year and/or have related chronic and complex diseases affecting their ability to live independently and successfully.
Job Responsibility:
Functioning as part of the interdisciplinary health care team to assist in meeting basic needs of patients in accordance with the policies, procedures, and standards of Riverside Health Care Facilities
Carry out all work assignments safely and be knowledgeable or in compliance with relevant regulations, policies, and procedures
Seek assistance and guidance when necessary, observing patient’s needs and self-limitations
Support corporate rehab quality improvement initiatives
Exercise behaviour consistent with Riverside Health Care’s Code of Conduct

Demonstrate a tactful, empathetic, and caring patient approach, and encourage independence
Carry out treatment plans/document as designed by the therapist
Monitor any changes to a patient’s health status and report to the appropriate therapist
Booking appointments, making phone calls, connecting patient with community resources
Delivers patient care according to the philosophy, goals and objectives of the Rehabilitation Services department
Participates in departmental and related committees/meetings when required
Participates in educational activities and programs for patients
Reports any untoward / unusual incidents accurately and promptly to the therapist
Participates in the department’s quality management program
Other such duties or responsibilities as may be delegated.
Requirements:
Graduate of a PT/OT/Rehabilitation Assistant Diploma program, equivalent and/or combination of experience may be considered
Excellent organizational and time management skills
Excellent multi-tasking skills and the ability to work well under pressure
Proficient computer skills
Experience with electronic documentation
Demonstrated strong written and verbal communication skills
Demonstrated ability to maintain strict confidentiality
Demonstrated ability to function effectively independently, within a team, with front line personnel, and management
Must be able to follow verbal and written orders
Physically capable of frequent bending, stretching, and lifting and of assisting heavy-needs patients with bed mobility, transfers, ambulation and exercise program
Class G drivers license, access to reliable vehicle with and the ability to travel to various locations within catchment area.
