Conduct intake assessments and develop individualized case plans tailored to client needs and goals
Provide ongoing case management services to individuals and families experiencing homelessness or housing insecurity
Assist clients with accessing housing resources, emergency shelter, public benefits, healthcare, mental health services, substance abuse treatment, employment support, and community programs
Maintain regular contact with clients to monitor progress, provide advocacy, and adjust service plans as needed
Support clients with housing applications, documentation collection, referrals, and appointment coordination
Collaborate with local agencies, landlords, shelters, healthcare providers, and community partners to ensure continuity of care
Perform crisis intervention and de-escalation when necessary while maintaining professionalism and empathy
Maintain accurate and timely case notes, client records, and program documentation in compliance with agency and funding requirements
Track client outcomes and ensure compliance with grant, county, and program guidelines
Participate in outreach efforts, community engagement activities, and team meetings
Educate clients on life skills, budgeting, employment readiness, and housing retention strategies
Maintain confidentiality and uphold trauma-informed and client-centered care practices
Requirements
Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field preferred
Minimum 1–2 years of experience in case management, social services, housing navigation, homeless services, or community outreach preferred
Experience working with unhoused populations, mental health, substance abuse recovery, or underserved communities strongly preferred
Knowledge of local housing programs, public assistance resources, and community-based services
Strong communication, conflict resolution, and organizational skills
Ability to manage a high-volume caseload and prioritize effectively
Proficient in Microsoft Office and case management databases/systems
Bilingual Spanish is a plus
Valid California driver's license and reliable transportation may be required
