Job Responsibility:
Support clients across Burnie-Smithton with care that’s truly personal
Work closely with clients, their families and healthcare professionals to create tailored plans that fit their lives, goals, and routines
Help people stay independent and feel their best
Requirements:
Certificate III or higher in Case Management, Aged Care, Disability, Community Services, or a related field
Previous experience in case management within aged care, disability, or youth services
Excellent communication and interpersonal skills
A valid driver’s licence and access to a reliable vehicle
Confidence to work autonomously while collaborating with internal and external stakeholders
