As a Case Manager, you’ll support clients across Illawarra with care that’s truly personal. Requirements:
Certificate III or higher in Case Management, Aged Care, Disability, Community Services, or a related field
Previous experience in case management within aged care, disability, or youth services
Good understanding of Home Care Packages and the new Support at Home program
Excellent communication and interpersonal skills
A valid driver’s licence and access to a reliable vehicle
Confidence to work autonomously while collaborating with internal and external stakeholders
