We are looking for a compassionate and organized Case Manager to support adults in San Francisco, California who are transitioning from homelessness into stable hotel-based housing. This is a Contract position focused on helping residents maintain housing, access essential services, and improve overall well-being through consistent, client-centered support. The ideal candidate brings strong administrative skills, sound judgment in crisis situations, and the ability to build trust with individuals facing behavioral health, substance use, or medical challenges.
Job Responsibility
Oversee a high-volume caseload of residents and provide individualized case management designed to promote housing stability and personal progress
Meet with tenants regularly through outreach, intake discussions, and ongoing check-ins to evaluate needs, update goals, and coordinate support plans
Foster positive resident relationships through consistent engagement, problem-solving, and timely interventions that help prevent housing disruption
Guide tenants in securing and maintaining public benefits, and assist them with meeting recurring rent obligations
Address unit-related concerns by working alongside property staff during inspections, pest control visits, and habitability follow-up efforts
Connect residents with community resources such as healthcare, behavioral health treatment, employment services, and other supportive programs, while encouraging successful follow-through
Organize tenant activities, group events, and community-building opportunities in partnership with colleagues and property management teams
Respond to urgent tenant situations with calm de-escalation techniques, document services accurately, and maintain complete and confidential case files
Coordinate with internal departments and external service providers, report suspected abuse or neglect as required, and participate in scheduled meetings and agency initiatives
Requirements
Experience providing administrative support, case coordination, or direct service assistance in a human services, housing, or community-based setting
Ability to manage detailed documentation, data entry, filing, and record maintenance with a high degree of accuracy and confidentiality
Strong interpersonal skills with the ability to engage respectfully with individuals experiencing mental health, substance use, or medical challenges
Comfortable handling crisis situations with professionalism, sound judgment, and effective de-escalation techniques
Capable of balancing multiple priorities while managing a large caseload and meeting deadlines in a fast-paced environment
Proficiency in collaborating with multidisciplinary teams, property staff, and outside service providers to support client outcomes
Clear written and verbal communication skills, including the ability to prepare case notes and maintain organized client records
