We are looking for a dedicated Case Manager to join our team in San Francisco, California. In this contract position, you will work closely with tenants to provide essential case management services, promote housing stability, and improve their overall quality of life. This role requires a proactive approach to addressing tenant needs, fostering community engagement, and collaborating with various stakeholders.
Job Responsibility:
Manage a caseload of 60-90 tenants, providing tailored support for individuals dealing with mental health challenges, substance abuse, and medical issues
Conduct outreach visits upon tenant entry to housing, as well as ongoing assessments and follow-up visits to address their evolving needs
Develop and implement strategies to help tenants retain their housing through regular engagement and effective interventions
Assist tenants with accessing benefits, ensuring rent payments, and addressing unit habitability concerns
Organize community-building activities, such as tenant groups and social events, to foster a supportive environment
Refer tenants to appropriate external services, ensuring they receive necessary resources and support
Respond promptly to tenant crises, providing de-escalation interventions and emotional support
Maintain accurate and organized case files, including creation, updates, and archival, while ensuring confidentiality
Collaborate with internal departments and external providers to advocate for and meet tenant needs
Report incidents of abuse or neglect to supervisors and relevant authorities in a timely manner
Requirements:
Bachelor’s degree or at least 2 years of experience providing case management services to homeless or formerly homeless populations
Minimum of 1 year of experience working with diverse, low-income, or homeless individuals
Proven experience in case management and outreach
Familiarity with San Francisco-specific resources for homeless and formerly homeless individuals
Ability to manage organized case files for a large caseload of tenants
Proficiency in Microsoft Office Suite and strong documentation skills
Excellent problem-solving abilities and attention to detail
Strong interpersonal skills to connect with tenants and collaborate effectively with team members
