Job Description:

We are looking for a dedicated Case Manager Team Lead to oversee and support a team of case managers in delivering compassionate and effective services to individuals and families facing challenging circumstances. Job Responsibility:

Lead and provide guidance to case managers, ensuring they meet program objectives and deliver exceptional client care
Oversee clinical and operational aspects of service delivery, including client intakes, assessments, and tailored service plans
Ensure all case documentation is accurate, timely, and adheres to program regulations and reporting standards
Collaborate with internal teams and external community partners to coordinate comprehensive care for clients
Monitor staff workloads, conduct regular case reviews, and support quality assurance initiatives
Participate in hiring, onboarding, and performance evaluations for case management personnel
Assist in tracking program data, preparing reports, and contributing to program development and improvement
Handle escalated or complex cases, providing direct intervention and support as needed
Facilitate training and development opportunities for team members to enhance their skills and knowledge

Requirements:

Minimum of three years of experience in social services, mental health, housing programs, or a related field
Master’s degree in social work, psychology, or a related discipline (license or license-eligible preferred)
Proven ability to lead and mentor teams in a detail-oriented environment
Strong knowledge of trauma-informed care principles and practices
Familiarity with tools such as Atlassian Jira, CRM systems, and ERP platforms
Experience with Agile Scrum methodologies and budget management processes
Excellent communication skills and ability to collaborate effectively with diverse stakeholders
Proficiency in cloud technologies and systems like IBM or similar platforms
