Job Description:

Field Case Manager RN position helping dual eligible Medicare and Medicaid populations with complex health and social challenges. Responsible for assessing, planning, implementing, and coordinating case management activities to evaluate medical needs and facilitate member wellness.

Job Responsibility:

Assessing, planning, implementing, and coordinating all case management activities
Developing proactive plan of care to address identified issues
Using clinical tools and information/data review to evaluate member's needs
Applying clinical judgment to incorporate risk reduction strategies
Conducting assessments considering information from various sources
Using holistic approach to assess need for clinical referrals
Collaborating with supervisor and key stakeholders
Utilizing case management processes in compliance with regulations
Utilizing motivational interviewing skills for member engagement
Visiting members in their homes to evaluate for waiver services

Requirements:

Registered Nurse with active MI state license in good standing
Confidence working at home/independent thinker
Ability to travel within designated geographic area
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Effective computer skills including navigating multiple systems and keyboarding
Proficiency with standard corporate software applications (MS Word, Excel, Outlook, PowerPoint)
Associates degree required
BSN preferred

Nice to have:

Care Management experience
Discharge planning experience
Home health care coordination experience
Certified Case Manager certification
