Job Responsibility
Conduct telephonic (and occasional in-person) assessments of member health needs
Develop and implement individualized care plans to improve outcomes
Identify and address medical, behavioral, and social barriers to care
Coordinate care across programs, providers, and resources
Review claims/history to support effective case planning
Engage members using strong communication and motivational interviewing skills
Participate in case conferences and collaborate with clinical teams
Requirements
Active, unrestricted multistate RN license (in state of residence)
Willingness to obtain additional state licenses (company-paid)
3+ years of clinical RN experience
1+ year experience using Microsoft Office and web-based systems
Ability to work scheduled hours with occasional evening rotation
Associate Degree in Nursing (required)
Nice to have
Case management experience in an integrated model
Bilingual (Spanish/English)
Strong technical/computer proficiency
Located within driving distance of San Antonio, TX (occasional onsite meetings)
BSN (preferred)
