Job Description:

The Case Manager is responsible for developing and implementing healthcare strategies to ensure quality care for members, particularly those in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Programs (D-SNP/FIDE).

Job Responsibility:

Conducting in-home assessments using comprehensive tools
Creating and managing individualized care plans
Coordinating services such as adult/pediatric daycare, personal care, and nursing facility placements
Collaborating with members, families, providers, and interdisciplinary teams
Ensuring cost-effective service delivery and accurate documentation
Mentoring new hires once proficient
Traveling 50–75% within assigned counties

Requirements:

Active and Unrestricted RN license in NJ required
Must reside in Somerset County NJ
3+ years clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required
Successful completion of the NJ Choice certification is a contingency of employment
Must possess reliable transportation and be willing and able to travel up to 50–75% of the time

Nice to have:

Case management and/or discharge planning experience
Managed Care experience
Crisis intervention skills
