Job Description:

This Case Manager Registered Nurse position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based position out of Sangamon County Illinois. The requirements is for candidates to hold an Illinois Registered Nurse License, and travel up to 75% of the time to meet with members face to face.

Job Responsibility:

Coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness
Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration

Requirements:

Minimum 2+ years of clinical experience required
Active and unrestricted RN license in the state of IL
Willing and able to travel 50-75% of their time using their own vehicle to meet with members face to face in Sangamon County, IL and surrounding areas
Valid drivers license required
Reliable transportation required

Nice to have:

Case management and discharge planning experience
Managed Care experience
Microsoft Office Suites experience
