Job Description:

Field-based Case Manager position responsible for conducting home visits and comprehensive assessments for members enrolled in Managed Long-Term Services and Supports program (MLTSS) and/or Dual Special Needs Program. Requires advanced clinical judgment and critical thinking skills to facilitate appropriate physical, behavioral health, and psychosocial wrap around services.

Job Responsibility:

  • Develop, implement, support, and promote health service strategies, tactics, policies, and programs
  • Conduct face to face visits in members' homes utilizing comprehensive assessment tools
  • Coordinate and collaborate care with member/authorized representative, PCP, and care team participants
  • Schedule and attend interdisciplinary meetings
  • Advocate on members' behalf to ensure proper and safe discharge
  • Develop care plans and authorize services in a cost-effective manner
  • Document accurately and timely in member's electronic health record
  • Mentor new hires

Requirements:

  • Active and unrestricted RN license in NJ
  • 3+ years clinical practice experience (e.g., hospital setting, alternative care setting such as home health or ambulatory care)
  • Must live in Essex County NJ (preference given to candidates located in Newark, Irvington and South Orange)
  • Reliable transportation required
  • Successful completion of NJ Choice certification (exam score of 80% or higher)
  • Associate degree or Diploma in Nursing required

  • LTSS experience
  • Experience using computer, keyboard, mouse, multi-system navigation
  • MS Office Suite applications (Outlook, Word, Excel, SharePoint, Teams)
